TERMS AND CONDITIONS
All St. Mary's Medical Center mobile SMS programs run on the short code 89183.  St. Mary's Medical Center does not charge you to participate in this program; however, your wireless service provider may charge for sending and/or receiving SMS messages and air-time.

1. St. Mary's Medical Center provides a telephone call back service to schedule doctor appointments at St. Mary's Medical Center as part of a text messaging program (the Program).

   Users will text the keyword 'SEEME' to the shortcode 89183 to request a call from St. Mary's Medical Center to schedule an appointment.

   Please note that to process requests for this program, you may be charged a fee to send and receive SMS messages based on the terms of your wireless service agreement with your wireless service provider.  Other charges may also apply.  All charges are billed by and payable to your wireless service provider. Check your wireless service provider if you have questions about your service plan.

2. St. Mary's Medical Center will not be liable for any delays in the receipt of any SMS messages connected with this program, as delivery of SMS messages is subject to effective transmission from your wireless service provider/network operator.


3. The St. Mary's Medical Center program is available on all major carriers including Alltel, Boost Mobile, Cellular One, Cellular South, Cincinnati Bell, AT&T Mobility, Dobson, Midwest Wireless, Nextel, Sprint, Verizon Wireless, Virgin Mobile, and Cricket Wireless.


4. For additional questions and information about the St. Mary's Medical Center SMS program, please send an e-mail, including your question and your mobile phone number, to getadoc@chw.edu.

TO STOP (OPT-OUT)
You can cancel receipt of all St. Mary's Medical Center SMS messages, at any time, by sending STOP, END, QUIT or STOP SEEME to any text message you receive to the shortcode 89183.

PRIVACY POLICY
St. Mary's Medical Center respects your privacy. St. Mary's Medical Center will only use information you provide to this program to transmit your SMS messages, or as otherwise described in this document. Nonetheless, St. Mary's Medical Center reserves the right, at all times, to disclose any information as necessary to satisfy any law, regulation or governmental request, to avoid liability, or to protect our rights or property. By participating in the program, you agree not to use a false or misleading name or a name that you are not authorized to use. If St. Mary's Medical Center, in it's sole discretion, believes that any such information is untrue, inaccurate, or incomplete, St. Mary's Medical Center may refuse you access to the program and pursue any appropriate legal remedies.
